چکیده

The testes develop in the abdomen and descend during the third trimester into the scrotum. The testicle can be arrested anywhere during its descent or it can migrate into an abnormal position. Undescended testis, otherwise known as cryptorchidism, is a welldescribed disorder with a spectrum of complications ranging from torsion to necrosis, infertility, and atrophy. The risk of developing testicular cancer in an undescended testicle has been well-documented, as high as 32 times the general population if the testicle is not brought down before the age of 12. In clinical practice, surgical exploration for the testicle and definitive fixation in the scrotum is warranted at age two to three if it fails to descend to the normal position, with clinical success rate of 90%. However, the problem with an undescended testicle arises when surgical exploration is negative, and the usefulness of other approaches, including imaging tests, in this setting is not clearly established. We report a case of a 45 year-old man with history of undescended left testicle diagnosed at birth, and repeated negative surgical explorations presenting with symptomatic pelvic mass, and eventually diagnosed with testicular tumor. This case highlights the potential difficulty in establishing this diagnosis despite the generally favorable outcomes with orchiopexy. Given its high prevalence, testicular neoplasm must be included in the differential diagnosis of patients with a history of an undescended testicle who present with an acute abdomen. Surgical resection without pelvic lymph node dissection is the standard of care for an abdominal testicle found at exploration.

Cancer Risk in Mothers of Men Operated for Undescended Testis

BACKGROUND Undescended testis, or cryptorchidism, occurs in 2-5% of boys born at term, and by 12 months of age about 1% of all boys have manifest cryptorchidism. Several hormonal substances control this process and disruption of the foetal sex-hormones balance is a potential cause of undescended testis, however, to a great extent the aetiology of cryptorchidism is unclear. METHODOLOGY To stud...

Undescended testis and the risk of testicular cancer: importance of source and classification of exposure information.

BACKGROUND The strength of the association between undescended testis and testicular cancer varies considerably across studies. Here we report the effect of various classifications of self-reported history of undescended testis and different data sources on the estimates of the risk of testicular cancer from a case-control study. METHODS We performed a population-based case-control study incl...

Commentary to 'Management of undescended testes: European Association of Urology/European Society for Paediatric Urology Guidelines'.

CONTEXT Undescended testis is the most common endocrinological disease in the male newborn period. Incidence varies between 1.0% and 4.6% in full-term neonates, with rates as high as 45% in preterm neonates. Failure or delay of treatment can result in reduced fertility and/or increased testicular cancer risk in adulthood. OBJECTIVE To provide recommendations for the diagnosis and treatment of...

Undescended and Maldescended Testes

Retractile testes [1] Prepubertal boys can have an exaggerated cremasteric reflex. The testis may retract out of the scrotum in the cold, on examination, on excitement or on physical activity. It is normal and will descend when relaxed and warm, or it can be manipulated back into the scrotum. Retractile testes do not need any treatment but do need close follow-up until puberty, as they can beco...
